What to Bring to Your Appointment

Identification & Insurance

Be sure to bring your photo identification and medical insurance cards. Your co-pay will be collected at the time of service.

Medications & Record

Please bring an updated list of your medications along with your current medication bottles. If you monitor your blood pressure at home, bring your blood pressure cuff. If you track your weight, bring a record of a few recent weights. Additionally, if requested, bring any medical records from your referring physician.

Patient Forms

You will receive a link to complete your new patient forms prior to your appointment. If you do not receive this link, then you will be able to complete the forms at the time of your appointment.

Questions & Concerns

Please bring your list of questions or concerns with you to the appointment.

NOTE: Please refrain from using the restroom immediately before your visit as we will likely be taking a urine sample during your appointment.

Parking is allowed anywhere in the lot adjacent to the National Avenue building but there are several reserved spaces for Springfield Nephrology patients located on the south side of the building near an entrance and the elevators. Handicapped parking is also available on the west side of the parking lot.

If you are unable to walk, please call us at (417) 886-5000 from the lobby and we can ensure you have a wheelchair to use.

Utilize the elevators to go to the third floor where we are located.
